2. How We May Use and Disclose Your PHI
We may use or disclose your health information without your written permission in the following situations:
Treatment: To provide, coordinate, or manage your care.
Payment: To obtain reimbursement for services.
Healthcare Operations: For internal administration, quality assessment, and staff training.
Public Health and Safety: As required for reporting diseases, abuse, or threats to health and safety.
Legal Requirements: If ordered by a court or law enforcement agency.
Health Oversight: To comply with audits, investigations, or licensure reviews.
De-identified Information: We may use data that does not identify you personally.
Any other use or disclosure not listed above will only be made with your written authorization, which you may revoke at any time.
3. Your Rights Under HIPAA

Request Corrections: Ask for corrections to your health information if it’s inaccurate or incomplete.
Request Confidential Communications: Ask us to contact you in a specific way (e.g., only by phone, not mail).
Restrict Disclosures: Request limits on how we use or disclose your information (we may not be able to grant all requests).
Receive a List of Disclosures: Obtain a list of when your information was shared (excluding uses for treatment, payment, and operations).

File a Complaint: If you believe your rights have been violated, you may contact us or the U.S. Department of Health and Human Services. We will not retaliate against you.
4. Our Commitment to Data Security

5. Reproductive Health Privacy (2025 Update)
As required by new HIPAA rules effective in 2024, we offer additional privacy protections for health information related to reproductive health care. We will not disclose such information to law enforcement or other third parties without your authorization, except when required by federal law.
6. Breach Notification
If your PHI is involved in a breach, we will notify you without unreasonable delay and no later than 60 days after discovery. We will describe what happened, what information was involved, what we are doing to mitigate harm, and what you can do to protect yourself.
